Note: The forms above are for your reference. All updated forms will be filled out on your client portal and will need to be signed electronically before your first session. When you create a client profile and make your appointment, you will receive an email with a link to the required forms (intake, teletherapy consent, company policy consent, and the HIPAA/privacy policy form) that you will fill out/sign prior to your initial evaluation. I will offer in-person visits if the distance and my schedule allows.

Steps to Conduct Speech Therapy Virtually

Enjoy the convenience of teletherapy
1. Click Client Portal Login
After your phone consultation, I will determine if a speech-language assessment is needed. If it is needed, you will receive a welcome email with the 3 consent forms and the initial intake form. Please make a client account on the TheraPlatform, fill out your demographic information, input your card information for payment, and fill out all of the paperwork before we meet. Check your internet connection, headphones, and camera to make sure they are working properly. You will go to your client portal to find your appointment date/time. At the appointment time, go to your client portal by clicking on the link or you may find the client portal on my website on my menu. Sign in and you will see your appointment. You will hit the "Start Meeting" button to enter the virtual waiting room. When the therapist is ready, you will be admitted. Thank you for your patience.
2. Meet Together
I will conduct my assessment and we will schedule your follow-up with the testing results/1st therapy treatment session if you qualify.
3. Follow-up
You will receive a superbill via email if needed and the payment is due at the time of service. You will pay directly on your secure client portal using Stripe. You can schedule your future appointments directly on your client portal or we can set the next session up at the end of your treatment session. Please call or email if you have further questions.

Certifications and Licenses

I am ASHA Certified, licensed in California/Washington State to practise speech-language pathology, and I hold an Educational Staff Associates (ESA) certification. I am certified in the Simon Says Speech Therapy Programs for Orofacial Myofunctional Disorders. My therapy platform, called THERAPlatform, is HIPAA certified and is used for virtual therapy, scheduling, payment, superbills, and document collection.

Location
  • Generation Chiropractic in Port Orchard, Washington for in-person myofunctional therapy/evaluations and for speech/swallowing evaluations or treatment.
  • Teletherapy evaluation/treatment visits for California and Washington clients
  • Possible home visits for infants on a case-by-case basis and as the schedule allows. Call for more information!
